The problem

Organisations are deploying AI faster than they are governing it. Someone asks whether a project is "ready", "responsible" or "safe to launch", and the honest answer is a shrug backed by a slide. Nobody can say which controls exist, which have evidence behind them, which are missing, who owns the gap, or what specifically blocks approval. When something goes wrong — a biased outcome, a privacy exposure, a model nobody can explain — every one of those questions arrives at once, and the governance record has to answer them.
